HOW TO REGISTER AS A PROVIDER

Until we get a web form on the site, just send an email with the following information to cody@pugsandkellylive.com.

We need the following information:

• First & Last Name
• Name of your Company (if any)
• Which Category we should list you under
• Contact information (Phone Number & Email Address)
• Your complete Street Address
• A breif write up about your services.

Your street address will not be listed on the site unless you request it, but we do require it for our records.
